Performance Measure 6: Transition to Adulthood
Youth with special health care needs will receive the services necessary to make transitions to all aspects of adult life, including adult health care, work, and independence.
Why is this important?
Many organized services can help youth with special health needs transition to adulthood. These youth must be able to expect good quality health care, employment with benefits, and independence.
When youth and their families can easily access the services required for these benefits, the youth become much more prepared to take responsibility for their future health and financial needs, and to lead productive lives. Likewise, adult health care services must be organized in community settings designed for adults with special needs and must be delivered in culturally effective and consumer friendly environments.
