The HSC also designs and conducts studies focused on health care that inform the decisions and thinking of policy makers in both government and private industries.</p><br /><p>The HSC is currently undertaking one of these very studies, funded by the Commonwealth Fund, to examine the experiences of primary care practices using "care coordination agreements," sometimes called "service agreements," with other care providers (e.g., other medical practices, hospitals, urgent care centers).</p><br /><p>Researchers&nbsp;are looking for cases of agreements that have been put into place already, or will be within the first half of 2010. Any transition-related items are welcome for inclusion in the Digest. Community strategies to support transition are also important. These strategies can include topics such as pediatricians and adult providers receiving training to support transition, and youth representatives being included on local councils or coalitions. The D-PIP was in response to a July 2006 revised policy statement by the AAP on developmental surveillance and screening for children from birth to age 3.
