Community Recognition Program
Real-life stories of community successes in creating systems-of-services.
Dear Friends and Colleagues,
Champions for Inclusive Communities (ChampionsInC) needs your help to identify communities that are exemplary models of community-based service systems. As a DSCSHN National Center focused on achieving “community-based services that are organized so families can use them easily and are satisfied with the services they receive” (outcome #5), it is part of our mission to celebrate communities and acknowledge their efforts.
A community-based system of services is an infrastructure that operates across service sectors. It facilitates the integration of services where a variety of programs work together to meet the many, varied needs of children, youth, and families in the community. It includes the following key components:
- Supports to ensure that families partner in decision making
- Presence of medical homes that work in coordination with other services
- Early and continuous screening
- Adequate financing to ensure families receive needed services
- Services and supports are easily accessed, coordinated, and culturally competent
- Transition to adult life – including transition to adult health care providers as well as educational and independent living.
Recommend a Star Community: We know there are many communities in your area who are working toward achieving these outcomes and are making good progress. If you would like them to be recognized for their accomplishments, we ask that you please contact us with the name of the community and a local contact. (This should all be done with their permission, of course.)
The process: First, we will have a short conversation with the community contact person you recommend to learn more about the community. Based on the outcome of the initial conversation we may arrange an in-depth interview and talk with other stakeholders in the community.
How will ChampionsInC use this information? These model community-based service systems will be highlighted as part of a Community Recognition Program on our website, in the ChampionsInC eNewsletter, and through other national venues. Their accomplishments will provide other communities with ideas about how to integrate services and supports. The star communities can use the publicity in their own social marketing campaigns, so it's a win-win effort.
