Community-based service systems must be organized so families can use them easily and are satisfied.

What is a System of Services?

A system of services is a family-centered network of community-based services that is designed to promote the health and well-being of CYSHCN (Children and Youth with Special Health Care Needs) and their families.

Importance of Outcome 5

Many families of CYSHCN face frustration accessing services. Eligibility requirements, policies, procedures, and multiple locations can leave families feeling overwhelmed. There are often gaps in services due to agencies that provide limited services. Duplication in services from multiple coordinators and service plans may also occur. Needing to travel great distances to obtain specialized services may also be a challenge for families.

Key Components Checklist

  • Families have access to appropriate services and supports in their community.
  • Services are coordinated between all providers.
  • Families receive supports such as coordinated service plans or care coordinators.
  • Families are connected to necessary services via a streamlined enrollment process.
  • Families are satisfied with the services and supports they receive.
  • Partnerships work to develop service systems at the community level.

Working Together with Title V

Achieving Outcome 5 requires leadership from State Title V programs through:

  • Community-level care coordinators working with medical home and other programs.
  • State-level coalitions and councils that foster collaboration.
  • Coalitions and councils created to meet the unique needs of their community.
  • Culturally competent practices.
