Performance Measure 2: Access to Medical Home

Children with special health care needs will receive coordinated, ongoing, comprehensive care within a medical home.

Why is this important?

The key to organizing services so families of children and youth with special health care needs (CYSHCN) can use them easily is to implement a medical home. A medical home is a source of ongoing routine health care in a community. Ideally, parents and providers work as partners within a medical home to meet the needs of children and families. Care should be taken to initiate CYSHCN into conversations about their treatment while they are still in grade school.
