The six CSHCN performance measures now offer states a set of goals for children and youth with special health care needs and their families. However, state leaders are in need of methods to measure their ongoing progress.

Measuring & Monitoring Project: Assessment to Action
The Assessment to Action website was developed to assist states in creating a comprehensive system of care for children with special health care needs and their families as outlined in the federal 10-year action plan, Healthy People 2010. The M&M documents provided there include, among other things, the indicators crafted by teams from the participating states.
