Implementing in Communities
The natural result of a state plan for CYSHCN should be a community-based system of care where services are organized so that families can use them easily. Previous examples of planning and implementation with other agencies suggest that communities should consider and build upon their strengths. A unifying state plan will also help to provide support and guidance to the community.
