Care Coordination

Care coordination – also sometimes known as service coordination or case management - is the ongoing coordination of needed services for families of CYSHCN via a designated individual or team. ChampionsInC defines it as “a process whereby an individual or team of individuals assists a family in the identification, acquisition, and coordination of services to ensure the needs of the child and family are addressed in a comprehensive and efficient manner.”
