Performance Measure 2: Access to Medical Home

Children with special health care needs will receive coordinated, ongoing, comprehensive care within a medical home.

Why is this important?

Once identified, children with special health care needs require a medical home (i.e., a source of ongoing routine health care in their own community). Ideally, parents and providers work as partners to meet the needs of the child and family. Care should be taken to initiate youth with special needs into these conversations while they are still in grade school.
