This wheel (below) serves two purposes: it demonstrates that the 6 CYSHCN performance measures are interconnected and must be unified in order to implement a culturally competent, community-based system of services, and it demonstrates that every performance measure can be applied to the mission of every National Center. Here are some examples:

  • Youth and family leaders as partners on community coalitions
  • Focusing on health issues in transition planning with the help of schools, Voc Rehab
  • Creating community interagency screening clinics
  • Including a team of patients and families, primary care physicians, specialists and subspecialists, healthcare facilities, public health, and the community in medical homes.
  • Developing streamlined referral procedures for newborns who require follow-up with specialty care/community services.
  • Ensuring care coordinators can offer financial consulting and insurance information
  • Streamlining the application process to help families apply for multiple services in a non-duplicative manner
